1. How can I obtain copies or transfer copies of my - or my child's or guardian's - medical records?
(1) To receive a complete copy of your RediClinic medical records; or
(2) To have a copy of your RediClinic medical records sent to a health practitioner of your choice, please download and print a "Medical Request Form," fill out the form, and send it either of the following ways:
(a) To the address listed on the form; or
(a) To ensure a faster delivery, fax the Medical Request Form to 1-866-279-9592.

2. How long will RediClinic store my medical records?
The answer is fact and state dependent, but typically the records are stored for at least 10 years from the date of the medical visit.
3. How can I ensure that my medical records are protected?
As a health care provider, RediClinic will continue to abide by its "Notice of Privacy Practices," which was given to you the first time you visited RediClinic. This notice tells you how your information is used or disclosed and explains who has access to your information.
4. Who can I contact if I have anymore questions?
