2011-12 School Immunizations
Immunizations Dose Requirements to Enter Grades
K – 12*, to College
Diphtheria/Tetanus/Pertussis
(DTaP/DTP/DT/Td)
For grades K-6: 5 or 4 doses are required.
For students aged 7 years and older: 3 doses meet the requirement if one dose was received on or after the 4th birthday.
Tetanus/Diphtheria/Pertussis
(Tdap)
For 7th grade: 1 dose of Tdap is required, if 5 years have passed since last dose.
For 8th-12th grade: 1 dose of Tdap is required, if 10 years have passed since the last dose.
Polio
For grades K-12: 4 or 3 doses meet the requirement.
Measles, Mumps, and Rubella
(MMR)
For K and 2nd grade: 2 doses of MMR are required.
For 3rd-12th grade: 2 doses of a measles containing vaccine, and one dose each of rubella and mumps vaccine.
Hepatitis B
For students K-12: 3 doses are required.
For students aged 11-15 years: 2 doses meet the requirement if adult hepatitis B vaccine (Recombivax) was received.
Varicella
(Chickenpox vaccine)
For K-2nd and 7th-9th grade: 2 doses are required.
For all other grade levels: 1 dose is required.
For any student who receives the first dose on or after 13 years of age: 2 doses are required.
Meningococcal
(Bacterial meningitis vaccine)
For 7th and 8th grade: 1 dose is required.
College Students†, 1 dose is required.
Hepatitis A
For K and 2nd grade: 2 doses are required.

• Meningococcal – All College Students†
• HPV (Human Papilloma Virus) – Parents should consider this3 dose vaccine beginning at age 11 for all children
• Annual TB skin test
*A student shall show acceptable evidence of vaccination prior to entry, attendance, or transfer to a child-care facility or public or private elementary or secondary school in Texas.
