We are pleased to have served your immediate health care needs at Hackensack Meridian RediClinic inside select Rite Aids. As we continue to refine our Convenient Care network, we have made the decision to close our affiliated RediClinic locations and transition our patients to alternate Convenient Care locations such as Hackensack Meridian Urgent Care, Convenient Care NOW telemedicine and our  Hackensack Meridian Medical Group physicians.

For telemedicine, please visit ConvenientCareNow.com or search ‘Convenient Care Now’ in the app store to download the telemedicine app on your phone and initiate a video visit with a board certified physician. Using telemedicine instead of an office visit helps patients and caregivers avoid unnecessary exposure to further illness and allows for you consult with a board certified physician without having to leave your home.   During this time of COVID-19,  we are offering a discounted rate of $20, using promo code HMH20.  If you’re 65 and older, use promo code HMHFree for a free consult. Promo codes apply to NJ residents only.

How can I obtain copies or transfer copies of my-or my child’s -medical records?

(1) To receive a complete copy of your RediClinic medical records; or

(2) To have a copy of your RediClinic medical records sent to a health practitioner of your choice, please visit rediclinic.com/faqs/patient-medical-records to access a “Medical Request Form,” completely fill out the form, and send either of the following ways: (a) To the address listed on the form; or (b) To ensure a faster delivery, please fax the Medical Request Form to 1-866-279-9592.

What other rights do I have with respect to my protected health information?

Please review our “Notice of Privacy Practices”, which was given to you the first time you visited RediClinic, and is at rediclinic.com/privacy-policy. This notice provides you with important information, such as telling you how your information may be used or disclosed, what rights you have with respect to your information (e.g., requesting a restriction on our use or disclosure or an amendment to the information we have in our records), and how you may exercise these rights.
