To obtain or transfer copies of your, or your child’s, medical records please see below for instructions:

  1. To receive a complete copy of your RediClinic medical records; or
  2. To have a copy of your RediClinic medical records sent to a health practitioner of your choice, please visit rediclinic.com/faqs/patient-medical-records
    to access a “Medical Request Form,” completely fill out the form, and send to the address listed on the form or for faster delivery,
    please fax the Medical Request Form to 1-866-279-9592.
