  1. How do I find a new provider?

    To find a new provider, we suggest you use local hospital referral services or contact your health insurance company to locate another provider for any of your medical needs.

  2. How can I obtain copies or transfer copies of my—or my child’s —medical records?

    1. To receive a complete copy of your RediClinic medical records; or

    2. To have a copy of your RediClinic medical records sent to a health practitioner of your choice, please visit to access a “Medical Request Form,” completely fill out the form, and send either of the following ways:

      (a) To the address listed on the form; or

      (b) To ensure a faster delivery, please fax the Medical Request Form to 1-866-279-9592.

  3. What other rights do I have with respect to my protected health information?

    Please review our “Notice of Privacy Practices”, which was given to you the first time you visited RediClinic, and is also at This notice provides you with important information, such as telling you how your information may be used or disclosed, what rights you have with respect to your information (e.g., requesting a restriction on our use or disclosure or an amendment to the information we have in our records), and how you may exercise these rights.

  4. How can I ensure my medical records are protected?
