Put school and sports physicals on your back-to-school checklist!


Most schools require a physical examination before the year begins.

School. Pre-enrollment physicals may be required. Ask your child’s school about any required forms before your appointment.**

Sports. Most sports programs require pre-participation physicals.

Basic. A basic physical gives you a general look at the state of your health.

Camp. Pre-participation physicals are mandatory at most camps, both for campers and counselors.

At RediClinic, we’re here to ensure your child’s health and all their records are up-to-date, including required immunizations and this year’s flu shot.

Physical exams are an opportunity to address specific issues, including injuries, nutrition, training and exercise.

Full payment must be satisfied on date of visit.
**Each patient will receive a printed assessment from RediClinic with the results. Please be sure to bring all required forms related to the school and sports physical, including the completed health, immunization history and any required forms. These forms will be stamped by a RediClinic Clinician and will refer to the results listed on the RediClinic form.

Prepare for your Visit to RediClinic

Patients under the age of 18 must be accompanied by a parent or guardian

Bring your confirmation code for check-in
Please arrive 15 minutes prior to your appointment
Bring a valid government issued I.D.
If you are using insurance, bring a copy of your insurance card
Bring completed, required forms if your visit is a physical
